OCI (Adult) -- All documents below 2 copies except Additional Particulars Form
1) OCI online application - 1 Photo 2in x 2 in
2) Additional Particular Form
3) CKGS order form - 1 for each application, copies not required
4) Copy of US Passport
5) Copy of Naturalization certificate
6) Copy of Birth Certificate
7) Proof of Indian Origin (Unexpired indian passport copy)
8) Proof of Address - Driver's license / Utility bill

2) U Stamp on the US passport
